Provides social services to patients as needed.
The Medical Social Worker is responsible for providing comprehensive social work and care management to patients and families by utilizing holistic assessment and appropriate intervention strategies that promote improvement of psychosocial problems, efficient use of medical resources, and facilitate safe and appropriate discharge planning and care managment as it relates to specific medical disorders and psychosocial needs of patient and their family. In addition to regularly schedule hours, the Medical social worker provides social work services in hospital during after hours, weekends and holidays as part of the department on-call schedule. The social worker is under the supervision of the Director of Social Services.
Social Work license in the State of Idaho. Experience in medical or psychiatric social work preferred. Experience working with community resources, and at least 2 years working as a licensed social worker.
